Colin has served as Chief Resident and Assistant Program Director at UCLA, where he oversaw training in clinical medicine, patient advocacy, and health care quality. He practices patient-centered care, working to understand his patients holistically so they may live their healthiest and happiest lives.
Balem Choi is an adult gerontology primary care nurse practitioner based in Pasadena. Her clinical career has focused on hospice and palliative care, with prior work experience including clinical nursing at Memorial Sloan-Kettering Cancer Center and hospice and palliative care in New York and California. Balem holds a BA from Dartmouth College, a BS in Nursing from LIU Brooklyn, and a Master’s Degree in Adult Gerontology from NYU.
Dr. Magda Lenartowicz is a passionate physician specializing in the care of older adults. She is a trained internist with a fellowship in geriatric medicine and social scientist with a specialization in gerontology. Prior to joining WelbeHealth, she founded a home health company and taught gerontology at McMaster University in Canada. Most recently, she served as medical director at a comprehensive geriatric inpatient service in Southern California and continues her clinical work as a hospice and palliative care physician with Rozeroom Hospice. Dr. Lenartowicz received her geriatric fellowship training from University of California Los Angeles and completed her undergraduate medical training and residency at the University of Saskatchewan, Canada. She completed her undergraduate training in gerontology at McMaster University.
Dr. Germin Soliman, DO is an internal medicine specialist from Newport Beach. She graduated from Western University College of Osteopathic Medicine of the Pacific in 2001 and has been passionately serving seniors ever since.
As a Registered Nurse, Priscilla brings experience in rendering care and leading services and programs for the frail elderly in Adult Day Health Care, Home Health, and PACE settings. With a compassionate, deliberate and individualized approach, she remains positive for the future of our seniors and their loved ones.
Marilyn has been working with seniors and other vulnerable populations for nearly 30 years, including managing adult day health care/CBAS centers, senior centers, dementia programs, and in-home intensive case management. Her passion is ensuring seniors receive high quality services that truly meet their individual needs.
Brian has spent his 13-year healthcare career focused on improving the lives of Los Angeles’ most fragile and at-risk children and adults, connecting them with high-caliber home health and housing options. He is passionate about seeing seniors live out their best years of life in the comfort of their own homes.
Sophia is a passionate health care leader with an expertise in senior care operations. She was at the helm of AltaMed’s ADHC and PACE growth and expansion for over 18 years. She believes in loving PACE participants and developing future leaders.
