TAKING ACTION TO PROTECT OUR SENIORS DURING THE COVID-19 PANDEMIC

To our dear seniors, families, and caregivers,

As you know, the spread of coronavirus continues to escalate globally, nationally, and here in California. During these challenging times, I want you to know that our leadership teams have been working around the clock to implement a proactive clinical and operating model to keep our seniors safe and our team healthy.

We are taking bold, aggressive, and preemptive actions to maintain the health and well-being of our PACE participants and our team.

Pandemic Response Systems Implemented:

Our organization has been prepared to operate during a pandemic since long before the emergence of COVID-19. On March 13, we formally mobilized our emergency preparedness program and launched our Pandemic Incident Response Systems. These systems include a well-defined chain of command and pandemic-specific roles and responsibilities for our team, ensuring clear communication and enabling continued high quality of care for our participants.

Remote/At-Home Patient Care Launched:

As of March 16, our PACE participants are receiving nearly all their care remotely, at home. In addition to medical care, we continue to provide essential services to our seniors based on individual needs, including food preparation and delivery, medication delivery, and personal hygiene assistance.

Each participant has also been provided with a 4G LTE “WelbeLink” tablet with hardware and software specifically designed for use by seniors. Our care teams are using these tablets to maintain regular video communications with participants in the home.

Work from Home Mandated:

As of March 16, all WelbeHealth staff have been mandated to shelter in place and work from home unless otherwise assigned by our emergency response team. This mandate protects our participants and caregivers and ensures immediate containment of any single COVID-19 case.

COVID-19 Resources Ready:

We’re working closely with local, state, and federal health officials to ensure we have appropriate resources in place to care for our seniors. We are prepared to test participants and staff for COVID-19 when appropriate. In addition, we continue to monitor and comply with evolving government and CDC guidance.

My colleagues and I chose to work in healthcare out of a sense of calling—a desire to serve and contribute when it matters most. I’m grateful for the opportunity to serve you, and I assure you that our team will continue to make every effort to do the right thing and protect our seniors during this critical time.

Peter Xiao, MD

Medical Director

Dr. Xiao graduated from UC Davis Geriatric Fellowship in 2008. Prior to that he finished his Family Medicine Residency training from the University of Wyoming in Cheyenne and attended medical School at Chulalongkorn University. After completing his education, he became a Hospitalist. He worked in many hospitals, including Sutter Coast Hospital, Shasta Regional Medical Center, St. Joseph’s Medical Center, and Vibra Specialty Hospital of Sacramento before becoming the Hospitalist Medical Director at Mercy Merced Medical Center. Passionate with education, Dr. Xiao joined UCSF Fresno Family and Community Medicine Residency Program as core faculty and currently holds the academic appointment of Associate Professor. Bringing his wealth of knowledge and experience, Dr. Xiao, joins Welbe Health as the Medial Director of Sequoia Pace Program.

Nicole Butler

Center Director

Nicole has been a healthcare professional for over 20 years and has a strong record of achievements in growth, operations, marketing, sales, development, as well as planning and execution. She has worked on strategy, development and overall ways to improve the delivery of health to everyone.

Paola Pouponneau-Nisbett

Executive Director

Paola has an extensive work history in the health industry. She has worked in both the medical and mental health field, working with various populations. And has been able to work from direct care service to leadership roles.

Magda Lenartowicz, MD

Medical Director

Dr. Magda Lenartowicz is a passionate physician specializing in the care of older adults. She is a trained internist with a fellowship in geriatric medicine and social scientist with a specialization in gerontology. Prior to joining WelbeHealth, she founded a home health company and taught gerontology at McMaster University in Canada. Most recently, she served as medical director at a comprehensive geriatric inpatient service in Southern California and continues her clinical work as a hospice and palliative care physician with Rozeroom Hospice. Dr. Lenartowicz received her geriatric fellowship training from University of California Los Angeles and completed her undergraduate medical training and residency at the University of Saskatchewan, Canada. She completed her undergraduate training in gerontology at McMaster University.

Rosana Scolari

SVP, Operations and Executive Director

A dynamic and team-oriented senior-level professional with 23 years of experience leading and developing healthcare programs in a non-profit health care/social service setting. Recognized for goal-oriented organizational leadership and successful track record of delivering high quality services and positive customer satisfaction.

Jessica Lopez

Center Director

Jessica’s passion lies in caring for our elders, and she deeply values the integrity of our most vulnerable seniors. She has spent over 13 years providing comprehensive healthcare services to our geriatric patients as a leader in PACE.

Kendell Cannon, MD

Medical Director

Kendell has served as both a Teaching Attending and a Hospice Medical Director. She recently completed a prestigious fellowship at Stanford University with a focus on “Redesigning Care for People in Late Life.” She believes in focusing on her patient’s life goals to maximize their health and well-being.

Jillian has spent her career focused on improving geriatric services with an emphasis on patient-centered care. As a veteran PACE and adult day health care executive, she is passionate about promoting health, independence, and an overall increased quality of life for seniors and their families.
