WelbeHealth Achieves Prestigious IHI Level 2 Designation
We are thrilled to announce that WelbeHealth has earned Level 2 recognition from the Institute for Healthcare Improvement (IHI) as an Age-Friendly Health System for our first seven PACE centers. This prestigious designation highlights our ongoing commitment to providing exceptional, age-friendly care to our senior participants.
What Does This Mean?
The IHI’s Age-Friendly Health System initiative is a nationwide effort to ensure that older adults receive evidence-based care that aligns with what matters most to them. Achieving Level 2 recognition means that our centers have successfully integrated the “4Ms” framework—What Matters (to older adults), Medication, Mentation, and Mobility—into our care practices, ensuring that our participants receive the highest quality of care that is personalized to their needs.
Dr. Matt Patterson, President of WelbeHealth, emphasized the importance of this achievement:
“Receiving Level 2 designation is a testament to our team’s commitment to our seniors, their teams, and their dedication to providing care that honors the dignity and autonomy of our senior participants. We see this play out every day as our team members help participants live how they want to live, and when our participants tell us that our team makes them feel like they matter.”
Unique in Our Geographies
Welbe is among an elite group of only 16 PACE organizations nationwide to achieve Level 2 status and our California centers are particularly distinguished, as we are the only PACE provider in the state to hold this recognition in our service areas.
Dr. Michael Le, Chief Medical Officer at WelbeHealth, highlighted the significance of this distinction:
“This recognition affirms that our approach to care is not only compassionate but also effective. By focusing on the 4Ms, we ensure that our participants receive care that is tailored to their needs, promotes their well-being, and supports their independence. We are also fortunate to be able to deliver this highly impactful care through a valued based and personalized care model.”
The Impact of Age-Friendly Care
Achieving Level 2 recognition is not just about earning a badge—it’s about the tangible benefits that our participants experience. Studies have shown that organizations recognized as Age-Friendly Health Systems see improvements in patient satisfaction, care outcomes, and overall quality of life. Participants feel heard, respected and empowered in their care decisions, leading to better alignment between their treatment plans and what truly matters to them.
As Kelly McDonnell, Vice President of Corporate Strategy and Services at WelbeHealth, noted, this recognition sets us apart:
“We are proud to be leading the way in age-friendly care in our communities. This achievement underscores our commitment to continuous improvement and excellence in care delivery.”
Looking Ahead
WelbeHealth’s commitment to providing age-friendly care is ongoing. As we continue to expand our services and reach more seniors in need, we remain focused on ensuring that every participant receives care that is respectful, personalized, and aligned with their values. This designation is just the beginning, and we are excited to continue our journey of care excellence.
Thank you to our incredible team members who have made this achievement possible. Your dedication to our mission and to the well-being of our participants is truly inspiring. Together, we will continue to positively impact the lives of the seniors we serve.
