Dear Assisted Living Facility Partners,
To help streamline our coordination process and improve turnaround times, WelbeHealth will be sunsetting the use of FADA forms for Assisted Living Facility (ALF) partners beginning August 17, 2026.
What’s changing?
Starting 8/17/26, ALF partners will no longer receive FADA forms for:
- Participants transitioning into your facility
- Care level updates for existing participants
Instead, WelbeHealth will send a Referral Order via Fax to your facility for each participant requiring services.
Referral Orders will include:
- Participant information
- Diagnosis code(s)
- Effective Date
- Authorized Tier Level for services being referred
Why this change?
This update is intended to:
- Increase efficiency and reduce administrative burden for all parties
- Simplify and standardize our process
- Align ALF workflows with WelbeHealth’s broader provider network process, where services are coordinated through referral orders
- Eliminate duplication, as service expectations and rates are already outlined within your existing agreement with WelbeHealth
What ALF partners should expect:
- Referral Orders for services will begin being issued on 8/17/26
- FADA forms will no longer be emailed after this date, please look out for Faxed Referral orders
- Please reference your WelbeHealth agreement for applicable rates and service codes
- Continue coordinating participant care with the assigned RN Case Manager (RNCM)
- Ensure your facility’s fax number and contact information are current to avoid delays in communication
