What is PACE?

The Program of- All-Inclusive Care for the Elderly (PACE) is a coordinated healthcare program designed to support older adults who require a high level of medical care but wish to remain living in their homes and communities.

PACE brings together a wide range of healthcare and supportive services into one integrated program.

Instead of receiving services from multiple separate providers, participants receive care through a coordinated system managed by a dedicated care team. This approach helps ensure that medical treatment, supportive services, and social care are organized around the individual needs of each participant.

The program is structured to provide comprehensive care that may include medical services, therapy, medications, social services, and assistance with daily activities. Many services are delivered through a PACE center, in a participant’s home, or through other healthcare settings as needed.

PACE is designed to help eligible older adults receive the care they require while supporting independence and quality of life in a community setting.

Who Qualifies for PACE?

Eligibility for the PACE program is determined based on specific criteria established by federal and state guidelines.

In general, individuals may qualify if they:

  • Are 55 years of age or older
  • Live within the service area of an approved PACE organization
  • Are determined by the state to require a nursing home level of care
  • Are able to live safely in the community with the support of PACE services at the time of enrollment

Eligibility assessments are typically conducted by healthcare professionals who evaluate an individual’s medical condition, functional needs, and overall care requirements.

Enrollment in PACE is voluntary, and participants have the right to choose whether the program is the appropriate care option for their situation. PACE participants may disenroll from the program at any time, for any reason. Those with Medicare or Medicaid who disenroll will be assisted in returning to their former health care coverage.

How Does PACE Work?

PACE operates using a team-based care model designed to address the medical, physical, and social needs of each participant.

Interdisciplinary Care Team

Participants receive care from a group of professionals who work collaboratively to manage their healthcare needs. This interdisciplinary team may include physicians, nurses, therapists, social workers, dietitians, and other healthcare specialists. The care team evaluates each participant’s health status and develops a personalized care plan that outlines the services and support needed.

Individualized Care Plans

Care plans are developed based on a participant’s medical conditions, daily living needs, and personal goals. The interdisciplinary team regularly reviews and adjusts these plans to ensure that services remain appropriate as needs change.

Coordinated Services

PACE organizations coordinate all medically necessary services approved by the care team. Services may be provided in different settings depending on the needs of the participant, including:

  • PACE centers
  • The participant’s home
  • Hospitals or specialty care facilities
  • Skilled nursing facilities when required

This integrated approach allows healthcare providers to monitor health conditions closely while helping participants maintain independence whenever possible.
