WelbeHealth’s Program of All-Inclusive Care for the Elderly (PACE) coordinates the care of each enrolled participant based on their unique needs, with the goal of helping them remain living at home in their community.

The Program of All-Inclusive Care for the Elderly, or PACE, is a Medicare and Medicaid program that helps people meet their healthcare needs in the community instead of going to a nursing home or other care facility. PACE is both a primary care provider and a health insurer. Currently, there are more than 240 PACE centers operating in more than 30 states. WelbeHealth delivers PACE services with quality and compassion. We do this through our network of committed providers who deliver care with kindness.

Enrollment is year-round. You can enroll or disenroll any time of the year.
Yes, we bring you to and from specialist appointments and visits to our center.
Your center visits will depend your needs and your individual care plan. We host activities and socializing events each day, along with no-cost meals and snacks.

We serve breakfast, lunch, dinner and snacks. If your doctor recommends a special diet, we will create a meal plan that works for you.

We provide in-home care if you need it for medical reasons and can’t come to the center. We assess your needs and create a personal care plan for your situation.

Eligibility

To qualify for PACE, you must meet the following criteria:

  • 55 years of age or older
  • Live in the designated service area
  • Certified by the state as eligible for nursing facility level of care
  • Live safely in your home and the community

Enrollment in the PACE program is voluntary. You will stay enrolled for as long as you want to be, regardless of changes in health status.

This varies by state. In California, a doctor determines if there’s a need for nursing home care. People may need nursing home care if they can’t self-administer medications or feed themselves, or if they need help dressing/bathing or suffer from a mental illness or Alzheimer’s.

Insurance / Costs

Medicare and Medi-Cal reimburse WelbeHealth for care. If you are enrolled in Medicare and Medi-Cal, you will pay nothing, or you will pay your Medi-Cal “share of cost.”

Once you enroll, WelbeHealth coordinates and pays for all your medical care, becoming both your insurer and provider.

The cost for WelbeHealth services depends on your financial situation. Most participants receive our services at no cost to them.

  • If you have Medi-Cal or both Medicare and Medi-Cal with no share of cost, then our services are at no cost.
  • If you are eligible for Medicare but not Medi-Cal, you pay Medi-Cal and Medicare Part D premiums.
  • If you are not eligible for Medicare or Medi-Cal, you pay the self-pay rate.
When you enroll, you keep Medicare and/or Medi-Cal benefits, along with added benefits offered by WelbeHealth.

Enrollment

First, we’ll call you to explain our program. Then we’ll schedule a tour of the WelbeHealth center and do a home care assessment.

If you move outside of WelbeHealth service areas, we’ll try to transfer you to a different PACE center in your new location.

We will help you choose a primary care physician in our network. In some cases, we can arrange for visits with your current doctor as well.

It usually takes about two weeks to apply. Once your application is approved, you can start at the beginning of the following month.

If you’re not satisfied, you can leave at the end of the month to be enrolled in Medicare and Medicaid programs. Notify us and we can arrange the transition.
