Our Prior Authorization List has been updated to reflect changes that are effective August 01, 2025.

Some of the changes to the prior authorization list are:

• 10 hospice codes
• 17 COVID-19 testing codes when billed at POS 27, 81, 82–89, or 99
• 163 wound care codes
• 6 in-office injection (J) codes
• 40 dental imaging codes

As a reminder….

WelbeHealth has a subset of services that require prior authorization. If a service being provided requires authorization, it must be obtained prior to services being rendered.
